  • No controlled medications

  • This plan is for ages 6 months and up

• Strep and Flu Swabs, UA

• Up to 2 injections (+$10)

• Written Rxs only

• No narcotics or controlled meds

$65 UNLIMITED PRIMARY CARE VISITS

  • This plan is for ages 6 months and up
